Video Summary
The webinar explained how the pharmacist patient care process (PPCP) supports consistent, patient-centered clinical pharmacy care and how it should be taught to students and residents. Dusty Leacy reviewed the history of pharmaceutical care and the Joint Commission of Pharmacy Practitioners’ effort to create a standardized process. The PPCP includes five steps: collection, assessment, plan, implementation, and follow-up monitoring/evaluation. These steps help pharmacists gather patient information, evaluate medication therapy, develop individualized care plans, carry out interventions, and monitor outcomes.

The presentation emphasized integrating PPCP into all practice settings and educational levels. IPPE students typically begin with data collection, while preceptors complete the more advanced steps. APPE students are expected to participate in more of the full process, with increasing independence over time. Residents should use all PPCP steps routinely and receive individualized development based on baseline skills. Examples included medication history, counseling, rounding, drug information, antimicrobial stewardship, anticoagulation, renal dosing, and interprofessional collaboration.

Overall, the webinar stressed that PPCP is a clear framework for improving patient outcomes, standardizing pharmacy practice, and preparing learners to become effective future practitioners and preceptors.
